Intake process and information needed
All referrals and inquiries begin with a call to our Admissions Coordinator, Heather McMahon, LMHC: 877.845.5235. Heather is a Licensed Mental Health Counselor and a specialist in client intake and assessment.
On this call, Heather will obtain pertinent clinical and demographic information, and begin the assessment process. She will also request that certain records be forwarded to her for clinical review, including:
- Most recent psychiatric evaluation
- Most recent history and physical
- Most recent psycho-social history
- Most recent clinical notes (if in a treatment program)
- List of current medications (dosage and frequency)
- Other clinical or educational records that may be useful
All potential clients are encouraged to complete our Online Application for treatment. This application may be completed by the client, a parent or a professional referent.
Once the clinical information has been reviewed by our Admissions Coordinator and/or Clinical Manager, a determination of viability will be given.
For clients who are not accepted for treatment, a reason for the denial will be provided, as well as recommendations for other treatment options.
For clients who are accepted into our treatment programs, the parent or person responsible for financial payment will be forwarded to the Business Office to make payment arrangements. After this is completed, an admission date will be scheduled with the Admissions Coordinator.
Clients, parents and referents are encouraged to visit our programs at any time during this process.
