OASIS – Outreach and Social Integration Services aftercare

Our comprehensive aftercare service is called OASIS – Outreach and Social Integration Services. The aftercare program is a collaboration between the client, family, community professionals and our OASIS Coordinator. Through case management, OASIS is designed to improve the likelihood of success during the critical social integration phase after discharge.
