Case Management
The Transitional
Living and Learning Center offers intensive case management services
that are highly customized and designed to facilitate individual growth
and success. The ultimate goal of TLLC Case Management Services is to
help our clients become as highly functioning and productive as possible.
TLLC Case Management is provided by a team of dynamic members, including Case Managers and Care Managers, who collaborate with and coordinate services with outside professionals, agencies and organizations. Case Management focuses on linking clients with appropriate resources, providing skills trainings and classes, and providing outreach and support. The Case Management team provides a “safety net” for our clients by being able to identify issues and obstacles, and implement effective strategies to deal with them, before things get worse.
Case Management is not a time-limited service, but is intended to be ongoing, providing clients whatever they need whenever they need it, for as long as necessary.
Case Management is for clients who:
- Are leaving therapeutic settings, like: hospitals, residential treatment centers, wilderness
treatment programs, transitional living programs and therapeutic schools
- Are integrating back into the community, either through enrolling in college or entering the workforce
- Are capable of full independent living
- Are able to manage and administer their personal medications without assistance
